How to make a consent form?

i have to make a consent form for my research plan, i am using people over the age of 45 with cholesterol and high blood pressure. i need them to consent to me taking their cholesterol and blood pressure, for me using their ages and and test results, to consent to either using placebos or their regular medication, i need them to consent to using natural healing methods, also them not to eat fast food or fried foods. how to i conjure up this consent forms? thank you in advance
